Office Information: About Your Visit
New Patient Information
We welcome you to our practice! In preparation for your initial visit we ask that you download and complete our new patient forms and bring them with you to your appointment. Along with these forms we will need the following:
- All records relating to your current illness. This includes x-ray reports and original films, MRI reports and CDs and any other treatment notes or tests that may have been performed.
- Picture Identification
- Medical Insurance Cards
- A referral from your primary care physician (if your insurance requires one)
Insurance Information
We accept most major insurance plans.
Insurance Plan Information
Precision Orthopedics PC is not contracted with any insurance plans purchased on “The Market Place” web-site also known as “ObamaCare.” If you currently carry or have recently purchased any insurance plans offered under The Affordable Care Act please notify the office immediately to avoid any out of pocket expenses you will incur.
 Appointments
Precision Orthopedics is located in Brownstown, on Telegraph Rd., just south of Van Horn Rd. Please call 734-782-7003 to schedule an appointment. Appointments are scheduled Monday – Friday. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies, fracture add-ons or surgery. When physicians enter the exam room, they have no idea what multitude or complexity of problems they’ll encounter, nor the insurance hassles, paperwork, and phone calls they must endure to adequately care for the patient.
We apologize in advance if your wait time is longer than expected and we thank you for selecting our practice for your orthopedic needs.
Fees & Payments
We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, MasterCard, Visa, and American Express for your convenience. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient's responsibility. We will help in any way we can to assist you in handling claims.
Prescriptions & Renewals
Please request all prescriptions and authorizations for renewals during our regular office hours when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances.
Cancel / No-Show Policy
This policy has been established to improve appointment availability and access to care for our patients.
Parents are asked to call and cancel an appointment 24 hours prior to the scheduled date/time. A courtesy call to us should be made regardless of the time of the appointment. If we do not receive notice from the patient, the visit will be considered a no-show. Please keep in mind that missing or cancelling appointments at the last minute leaves other patients unable to be seen in a timely manner.
New patients who do not keep their first scheduled new patient appointment run the risk of NOT being re-scheduled. Exceptions to this policy must be approved by a provider or by the office manager.

Patients who consistently fail to present themselves for scheduled appointments will be considered chronic no-show. Three cancels or no-shows could result in dismissal from Precision Orthopedics P.C.

Late Policy
We are committed to prompt service and will work very hard, barring emergencies, to stay on time. We may ask you to reschedule if you arrive for your appointment more than 20 minutes after your scheduled appointment. Time management in a physician office is often a frustration to patients, staff and providers. We realize this and ask for your help and understanding.
